Qld mines stuck in 'fatality cycle': Report

Twelve Queensland miners are likely to die every five years unless the industry dramatically overhauls the way it deals with safety, according to an independent report released on Thursday.

The report also raised the possibility that mining managements were skimping on tougher safety precautions to save costs.
