City Rd was reopened in both directions around 3.30pm but traffic remains heavy on City Road, Ferrars Street and York Street.
Emergency services and heavy haulage are still at the scene, but the truck has been removed.
Route 96 trams use the rail bridge but a Yarra Trams spokeswoman said services were only disrupted for a few minutes.
Drivers could consider taking York St and then Moray or Clarendon streets, VicTraffic said.
“(It’s) not too bad at the moment but as we head into the afternoon peak we want people to plan ahead and avoid the area,” a spokesman said.
A witness told Denis Walter on 3AW he saw it slam into the bridge this afternoon, just before a tram went over.
“I saw the truck hit it and watched the bridge wobble, I’m glad I wasn’t on that tram,” the witness said.
