A woman with a big beautiful smile and a warm heart to match is how a mother is being remembered after she was tragically killed in a head-on crash with a truck south of Brisbane.

Sophie-Marie Pacey died after her car and a truck collided on Middle Rd at Greenbank about 2pm on July 5 while she was driving home from work.

The Flagstone resident was the sole occupant of the car.

The truck driver, a man in his 50s, was taken to Princess Alexandra Hospital in a stable condition.

Mrs Pacey left behind three children and husband Dale Pacey, who posted an emotional tribute to his wife on social media following the crash.

“It’s with a heavy heart that I am sharing the news that Sophie-Marie (Pacey nee) Wemyss passed away yesterday,” Mr Pacey wrote yesterday.

“My best friend, soulmate and mother of my three beautiful children.

“We will miss you forever.”

Her sister, Janis Wemyss, started a GoFundMe page to help the family with funeral costs.

She said Mrs Pacey had the “biggest heart” and her three children were her whole world.

“Thankfully, she had an amazing life partner and even in heaven she will know that the children are going to be loved so much by us all,” she said on the GoFundMe page.

“She was the type of person who just made her kids share a room, so she could take in a stranger who was facing being homeless and invited them into her home.

“She is also the person, who just the other week, not only bought a homeless person breakfast, she sat with them, having breakfast with her daughter right beside her.

“She truly just wanted to help the world and was kind.

“There are no words to describe the pain.

“Sophie always reminded her children to say goodbye and to tell people that you love them, because you never know when it will be the last time.

“Her words have never been so true.”

She said her sister was not covered by WorkCover when the accident happened, even though she was on her way home from her job, because she stopped at a friend’s house to pick up some slippers.

She also did not have funeral insurance, which is why Ms Wemyss started the GoFundMe page.

At time of publication it had raised more than $8700 of a $30,000 target.

“I am wanting to raise money for her high school sweetheart and best friend, so he can cover the cost of the funeral and spent time with the children during this time,” Ms Wemyss said.

“I know it's a hard time for all, so please only give if you can.

“Also, please let this be a reminder to always take care of those you love, because you never know when you won’t get a second chance.”

Many of Mrs Pacey’s friends also paid tribute to her on Facebook after the tragedy.

“You were always quick with a compliment and (your) big beautiful smile,” Rachelle McIntyre wrote.

“The way (your) eyes lit up when you talked about Dale and (your) babies is imprinted forever.

“Always wanting to see everybody happy, you were such a positive bright star Sophie-Marie Wemyss (Pacey), love you.”

Julie Singh wrote that her heart was aching after the news of her death.

“You will be forever in my heart and bright in my memories. You beautiful soul,’’ she said.

“Lots of love to your family. Oh, I wish I could put my arms around you right now.”

The Forensic Crash Unit is investigating the crash. Police asked witnesses or anyone with relevant dashcam footage to contact them.
