Almost two years have passed since former QBE chief executive Pat Regan was unceremoniously sacked from his job after a review related to “workplace communications” found he had breached the code of conduct.

What exactly happened remains a mystery, given the company has declined to answer questions related to the mysterious incident(s?), and executives appearing before parliament have confessed to not knowing anything further than what has been publicly stated.
