It is unfortunate the grave failings in corporate governance at Ardent Leisure Group in the lead-up to the death of four people at Dreamworld in 2016 were not explored in more detail by Queensland coroner James McDougall.

McDougall's 279-page report into the tragic accident on the Thunder River Rapids Ride on October 25, 2016 highlights failures in regulation as well as in Ardent's staff training, record keeping, and its slack response to multiple safety incidents on the deadly ride.
