An emergency physician has revealed the moments after a horror crash left people trapped under a mini bus on a major NSW highway.

Police are investigating the circumstances around the major crash that left three children with serious injuries and several others hurt.

Traffic on the Hume Highway at Marulan came to a halt about 12pm on Tuesday when the minibus left the road and flipped, coming to a stop on a grass median strip.

The driver, a man believed to be in his 50s, and seven passengers were trapped for a short time before they were freed by other motorists.

Emergency services rushed to the scene, with NSW Ambulances calling in 11 resources, including two helicopters.

A NSW Ambulance spokesman said the man in his 50s was taken to Liverpool Hospital in a critical condition, however, while he does have “serious injuries”, he has since been stabilised.

A woman in her 30s was taken by intensive care paramedics on road to Canberra Hospital with a medical crew on board in a critical condition, however, an update is still unknown.

Four children were taken to Westmead Hospital, three of which had minor injuries. Another child was taken to Canberra Hospital.

NSW Ambulance doctor Tom Evens told Today on Wednesday the minibus had been travelling along the highway when it rolled over multiple times “and had found itself on its side”, trapping a number of passengers.

“There was a mixture of adults and children in the minibus, and some of those people had come out through the windows as the bus rolled,” Dr Evens said.

“And what was very impressive is that members of the public had stopped and seen what had happened, and had worked together to free those people from underneath the bus and had pulled them out and started first aid with some support from our control room, which is fantastic.”

Dr Evens praised bystanders who rushed to help, saying “we … would never expect people to put themselves in danger”.

“But, members of the public who were driving just behind the crash stopped safely, made an assessment, realised that there were people in really imminent danger,” he said.

“(They) then worked together to move this minibus, to lift it, and to release some of the children from beneath.”

A witness told 10 News about “12 or 20” bystanders “pushed the van up and we flipped it back over” to pull one of the trapped occupants.

Police have established a crime scene as investigations into the circumstances of the crash are underway.

Parts of the van remained strewn across the highway on Tuesday, as specialist police worked to determine exactly what went wrong.

All southbound lanes of the Hume Highway were closed at Marulan with diversions in place to direct traffic.
