The car flipped onto its roof near the weighbridge exit at Mount White just before 1.30pm today.
The 4WD and caravan had hit the rock wall beside the southbound lane and rolled on its side, closing two of three southbound lanes.
The driver and a male passenger were trapped in the wreckage following the crash.
CareFlight’s rapid response rescue helicopter was called to the scene shortly before 2pm, landing on the road to drop off a critical care team.
The woman remained trapped in the vehicle for more than an hour as rescue teams worked to extricate her from the wreck.
CareFlight’s specialist doctor and critical care paramedic worked with NSW Ambulance paramedics to assist the woman while she remained trapped.

She has since been airlifted to Royal North Shore Hospital with suspected neck and spinal injuries. She is in a serious but stable condition.
Her male passenger was taken uninjured to the same hospital via road ambulance.
Emergency services including NSW Fire & Rescue, Police, the Rural Fire Service, the RMS and a tow truck all worked frantically to clear the scene as holiday traffic built up.
All southbound lanes have now re-opened but traffic is still heavy. Motorists are being told to expect significant delays.
