A man has been killed in a head-on collision in Sydney’s south during a horror afternoon on the city’s roads and a terrible week of transport accidents around the state.

A man has been killed in a head-on collision in Sydney’s south during a horror afternoon on the city’s roads and a terrible week of transport accidents around the state.

Five crashes around Sydney this afternoon left commuters stranded in up to 13km of wet Friday gridlock.

A man in his 30s was killed instantly after his silver Ford Falcon veered over the grass median strip at the Camden Bypass and smashed into a van about 2pm.

The Camden Bypass remains closed while investigations continue into the cause of the fatal crash.

In the city, a woman has been arrested after refusing a breath test following a rear-end collision with another car on that was stopped in traffic on the Sydney Harbour Bridge.

She was travelling with two toddlers aged two and five when her purple Mitsubishi Mirage rear-ended a black four-wheel drive about 5pm.

Two lanes were closed in each direction on the Bridge but have since reopened.

However traffic was banked up into the Lane Cove Tunnel on the north side of the Bridge, while traffic to the south is queued across the Anzac Bridge into Rozelle.

A truck also flipped on the M4 Motorway about 3pm after it hit a crane on the roadside.

One of three eastbound lanes was closed and traffic was backed up 13km.

At the Camden Bypass near Narellan at 2pm, crews arrived to find a silver Ford Falcon had swerved into the northbound lane of the major arterial road and collided with a black van.

The male driver of the Ford, in his 30s, died at the scene.

Ambulance Inspector John Williams said he was killed instantly.

“The deceased suffered pretty significant whole body injuries,” he said.

The female driver of the van was also injured in the accident, which a police spokeswoman described as “a mess.”

Inspector Williams said that she was rushed to hospital after being cut out of the wreckage of her vehicle.

“The woman was trapped for a short period before she was extricated by emergency crews,” he said.

“Our initial concerns are for chest, abdominal and pelvic injuries.

“The female is currently under the care of emergency doctors at Liverpool Hospital.”

Police are investigating the circumstances surrounding the accident, but Inspector Williams said that wet weather may have played a part.

“I urge motorists to take extra care on the wet roads this afternoon.”

The crash is the latest incident in a horror week of transport accidents in NSW.

A motorcyclist remains in a critical condition after a head-on crash with a truck in south Sydney yesterday afternoon.

A man also died earlier today after a freight train bound for Sydney collided with his Toyota Hilux at a railway crossing in regional NSW.

A female passenger is currently fighting for her life in Lake Cargelligo Hospital.
