The 67-year-old Card was severely injured in the crash on Geelong Ring Rd at Lara on Saturday night.
The Herald Sun reported that a 57-year-old woman, who has since been identified as Mandy, died after a Ford Ranger collided with a BMW.
The driver of the BMW, a 23-year-old Lara man, and his passenger, a 19-year-old Lovely Banks woman were taken to hospital with minor injuries.
A statement from Victoria Police read: “Major Collision Investigation Unit detectives have charged a man following a fatal collision in Lara on 16 November.

“A passenger from the utility, a 57-year-old Grovedale woman, died at the scene.
“The driver of the utility, a 67-year-old Grovedale man, was taken to hospital.
“The Grovedale man has since been charged with culpable driving causing death and dangerous driving causing death.”
Card has been bailed to appear at Geelong Magistrates’ Court on 26 November.
Card is president of Geelong’s Past Players group, with the secretary of the Past Players and Officials Club Nick Walsh sharing their condolences.
“The past players and the club are deeply saddened at this news and the passing of Mandy,” he said.
“We are shattered and are sending our deepest condolences to the family.”
Card was recruited by the Cats from Morwell Football Club after a best and fairest winning season in 1976.
He debuted in round 8 of the 1977 season and won the Carji Greeves Medal for the club’s best and fairest in 1983.
Between 1977 and 1987, Card played 110 games and booted four goals.
On Tuesday, a Cats spokesman said: “Geelong Football Club mourns the passing of Mandy, partner of past player Ray Card.
“The Club extends its deepest sympathies to Mandy’s children, Ray, extended families and friends.”
