A beloved son and father with the ‘biggest smile’ has been identified after a tragic death in custody incident in Maryborough.

Rhys Gregory Zemek, 30, formerly of the Sunshine Coast, was on Wednesday being remembered as a man with a big smile who would be “so loved and so missed”.

Emergency services were called to a motel on Alice St in Maryborough just before midday Saturday to conduct a welfare check on Mr Zemek.

Rhys Gregory Zemek, 30, formerly of the Sunshine Coast, was on Wednesday being remembered as a man with a big smile who was “so loved and so missed”.

He was allegedly acting erratically and officers restrained him before he was medically sedated and taken by ambulance to hospital, where he experienced a medical episode.

Mr Zemek was declared dead a short time later.

The incident is under investigation by the Ethical Standards Command on behalf of the State Coroner and investigations will be overseen by the Crime and Corruption Commission.
