AN INDEPENDENT auditor has recommended Nexus implement 51 changes to safety procedures at the Toowoomba Second Range Crossing.
The report, sparked in February by Transport and Main Roads Minister Mark Bailey's anger at the site's safety record, revealed the project's workplace health and safety procedures needed an overhaul.
Auditor Presna was tasked to file the report and investigate a string of vehicle rollovers at the project.
The auditor said the safety procedures at the site were generally consistent with industry expectations, but recommended a raft of changes and reviews into its safety protocol after rollover incidents continued to rise last year.
"Nexus Delivery and Ferrovial Agroman should improve the quality, consistency and independence of (safety) investigations," Presna's report said.
"The review undertaken of the plant rollover incidents has indicated that a number of these were associated with deviations from the planned work.
"A number of plant rollovers continued to occur into 2017, with the major contributing factors to these being the varying risk profile of the site, terrain and deviation from planned activities."
The report found of the 18 plant rollover investigations available on file six were classified as category one incidents, four as category two incidents and five as category three incidents.
The CFMEU alleged on social media there had been 33 rollovers since construction began.
