A police investigation is underway after a serious head-on car crash on the NSW Central Coast.

Two people were hospitalised and emergency services took an hour to free one man from the wreckage following the collision on the Old Pacific Highway at Mount White at 7.15am on Sunday.

A utility and sedan were involved in a head-on crash.

The driver of the sedan, a 27-year-old man, was trapped for about an hour before he was freed and airlifted to Royal North Shore Hospital in a serious but stable condition.

A 19-year-old female passenger in the utility was also airlifted to hospital in a serious but stable condition.

The 45-year-old driver of the ute was questioned at Gosford Police Station and released pending further inquiries.

Two female passengers also suffered minor injuries.

The crash closed the Old Pacific Highway in both directions and motorists were being urged to use the M1 Pacific Motorway instead.

The incident is being investigated and the scene is being examined by the crash investigation unit.

Anyone with dashcam or mobile phone footage, or any other information, is being urged to contact Gosford Police Station or CrimeStoppers.
