A family is mourning the loss of mum Cath Groom after an ambulance failed to arrive at her home.
The 51-year-old made a triple zero phone call after feeling chest pains in her Queensland home.
The call sparked an urgent code one emergency, meaning the ambulance should have arrived within 15 minutes with lights and sirens.
However, an emergency vehicle never arrived.

Mum dies after ambulance failed to arrive
After waiting for 90 minutes for the ambulance to show up, "exhausted" Cath cancelled the service and went to bed, thinking she could sleep off her chest pains.
Devastatingly, her son Nicholas woke the next morning to discover his mum dead in her bed; she passed away during her sleep.
Queensland Ambulance Services (QAS) has confirmed they would be reviewing the situation comprehensively.
“Firstly, the Queensland Ambulance Service's thoughts and condolences go out to the lady's family and friends,” a spokesperson told 9News.
“The QAS was experiencing a high level of demand for service at the time of the 000 calls.
“QAS is undertaking a comprehensive review of the clinical and operational aspects of this case as well as referring to the coroner.”

Single mum hailed an “amazing mother, woman and friend”
Friends and family of Cath have paid tribute to their beautiful friend, who was much loved in her community.
Cath had raised Nicholas following the tragic death of her husband 18 years ago. While the teenager was only a month old when his father died, he now must live without parents.
But her death would have been “entirely preventable” if an ambulance showed up on time, a source told 9News.
“Rest In Peace, Sis, may you rest easy now with Dad and your life’s love and husband Warren,” a tribute read.
“You will be missed by so many,” said another. “Cath, you were a wonderful wife and mum and a very special work colleague,” wrote a third.
“Cath, you're an amazing mother, woman and friend,” another heartfelt comment read. “Heaven is a greater place for having you.”
