Seven cars and a truck have been involved in a serious crash on a major Sydney motorway, causing Friday night chaos.

A major incident has caused a kilometres-long traffic back-up on the M4.

A truck and seven cars were involved in the collision at Prospect at the Light Horse interchange.

There were 21 people assessed by paramedics after the crash, with three people transported to hospital, according to Ambulance NSW.

Fire and Rescue NSW officers worked to free a man who had been trapped in his van. He is believed to have suffered head and leg injuries and was taken to Liverpool Hospital in a serious but stable condition.

Another man was treated for minor head injuries and was taken to Westmead Hospital in a stable condition.

A woman was treated for minor head and hip injuries and was taken to Westmead Hospital in a stable condition.

The driver of the truck has been taken to hospital for mandatory testing, according to NSW Police.

The crash occurred at 4:40pm, just before peak hour on one of Sydney’s busiest roads.

There is now more than 11km of standstill traffic in a westbound direction as motorists attempt to head home for the weekend.

The heaviest affected areas are between the Cumberland Highway off-ramp and the Wallgrove Road off-ramp, which are closed in a westbound direction.

The off ramp at Reservoir Road is also closed.

Westbound motorists are being diverted at Cumberland Hwy and are advised to use the Great Western Hwy as an alternative route
