An 11-year-old girl has died in hospital more than a week after a horror five-way crash that killed two motorists on a busy highway northwest of Sydney.

Emergency services were called to the Great Western Hwy, about 15km from Lithgow, shortly before 1pm on December 29 after reports of a multi-car crash.

Police believe an Izusu ute, towing a motorcycle trailer, collided with a Toyota HiLux travelling in the opposite direction before hitting a Kia Carnival head-on.

The driver of both the Izuzu – Sydney father Jason McMahon, 42 – and the driver of the Kia – former council worker David Drozd, 42 – died at the scene.

On Saturday, police confirmed an 11-year-old girl, who had been in the Kia, had died shortly after midday while at The Children’s Hospital at Westmead.

A 41-year-old woman and four more children aged between five and 15 had since been released from hospital after suffering varying injuries in the crash.

In addition to the two cars, police said the Toyota went on to collide with a Hyundai Tuscan, causing it to leave the roadway and trapping its occupants inside.

The driver, a 55-year-old man, suffered multiple fractures and was airlifted to John Hunter Hospital, while his two passengers were also taken to hospital.

A Nissan Patrol, which was also towing a trailer and was travelling behind the Isuzu, was also struck the trailer during the crash.

The four occupants of the Nissan and the Toyota, and their drivers, aged 39 and 42, were also taken to hospital for mandatory testing,

An investigation into the circumstances of the crash is being conducted by the Metropolitan Crash Investigation Unit, with a report to be prepared for the coroner.
