Investigators claim not everyone involved in a horror triple-fatal crash in Perth may have been wearing seatbelts.

Two brothers have been identified among the three men killed when a blue Kia Stinger and white Audi A3 sedan collided head-on, allegedly at speed, on Sixty Eight Rd in Baldivis about 5.10pm on Wednesday.

Father of two Kody Bell, 24, was a passenger in the Audi, with his 21-year-old brother Will and two friends also seated inside the car at the time of the crash.

The two brothers and 24-year-old driver Jaziah Thomas died as a result of critical injuries.

Two other men rescued from the wreck remain in a critical condition in hospital.

It’s understood one was a 21-year-old man – possibly the only survivor on-board the Audi.

One was flown to Royal Perth Hospital and the other was taken by car to Rockingham General Hospital.

The 44-year-old woman driving the Kia Stinger received minor injuries.

WA Police Detective Senior Sergeant Adam Matson said police were examining if speed was a factor.

“There’s some suggestion not all occupants had been wearing seatbelts at the time of the crash.” Sergeant Matson said.

“The loss of one life on our roads is a tragedy when it happens, so to lose three members of our community from the one crash, including two brothers, it is hard to comprehend.”

He said dashcam vision had already been provided and was assisting in the investigation.

“The message is, in a lot of these cases, it gets down to human error, people making mistakes,” Sergeant Matson said.

“What we do ask is that people pay attention on the roads and just drive to the road conditions.”

Tamekah Locker posted a tribute to her late partner Mr Thomas on Facebook, calling him “the most amazing, loving dad and son.”

“It still doesn’t feel real and I keep waiting for you to just walk through the door and it breaks me apart to know your [sic] not going too [sic],” Ms Locker wrote.

“You were the perfect partner and perfect dad we are so lucky to have had you in our lives and we will never forget you and all of our memories.”

Local resident Dee, who was one of the first people to arrive at the scene after the crash, has opened up about the horror she witnessed.

She said she was first notified of the crash when she heard the deafening explosion of the cars impact.

“I was walking the dog home and saw this white vehicle go past rather quickly,” she told 6PR radio station.

“Then I heard this terrible explosion probably about 80 metres up the road.”

“I ran straight to the scene and it was absolutely horrific.

“It was like a war zone it was awful.”
