The horrific crash occurred around 4.30pm on Tuesday near the corner of Warne and Raymond streets in the town south of Dubbo in central western NSW.
The vehicle is understood to have been travelling at speed when it careened into the group, a female adult with the four children.
She has been airlifted to a Sydney hospital, while the two surviving children are being conveyed to hospitals.
NSW Police media told news.com.au that the driver of the vehicle who struck the five people on the road fled the scene and is still being sought by police.
Multiple emergency vehicles and four rescue helicopters attended the scene.
NSW Ambulance Superintendent Andrew DeGabriel said paramedics were confronted by a tragic scene, Nine News reported.
“These types of incidents are shocking for all emergency services and witnesses involved,” he said.
“We were confronted with an absolutely devastating scene in Wellington.
“It’s heartbreaking for this to happen to such a small community.”
