Naya Rivera’s young son has given heartbreaking new details about her final moments as the late Glee star’s full autopsy and investigative reports are released for the first time.

Rivera went missing while swimming off a boat in Lake Piru, north of Los Angeles, on July 8. Her four-year-old son Josey was found alone on the boat hours later, and after a massive search, Rivera’s body was found in the lake several days later.

In the investigative report obtained by People, Josey said that his mother had counted “1, 2, 3” as they jumped off the boat together to swim in the lake.

“She helped him onto the boat and then he then heard [Naya] yell ‘help’ and she put her arm in the air. She then disappeared into the water,” the investigative report states.

The report also outlines factors that may have contributed to Rivera’s mysterious drowning death, which was been ruled as an accident. The actress suffered from vertigo – a condition that would get worse when she was in water.

Rivera “would have vertigo to the point of vomiting, but she learned to control the symptoms with antihistamines,” the report states, as per People. She had previously sought hospital treatment for the condition.

According to Rivera’s death certificate, issued three weeks after her death, she drowned “in minutes”. No drugs or alcohol were found in her system.

Josey had previously told investigators his mother boosted him onto their rented boat after they had been swimming together before she “disappeared”.

Police believe Rivera, “mustered enough energy to get her son back onto the boat but not enough to save herself,” Ventura County Sheriff Bill Ayub said after her body was found.

Lake Piru has been the site of several drownings due to its dangerous conditions.

According to the Los Angeles Times, at least seven people drowned in the reservoir between 1994 and 2000.

Douglas West, Lake Piru’s parks and recreation services manager at the time of the 2000 article, counted about a dozen drownings during his 23 years there.

The Times noted that “most drowning victims were swimmers venturing into restricted waters.”

“The only thing I can emphasise is that if swimmers are around water, they need to wear a personal flotation device whether they know how to swim or not,” West told the newspaper.

“That will save their lives.”

An eerie prediction of her final moments before the tragedy, Rivera’s last post on Instagram shows the 33-year-old mum in a tight embrace with Josey, simply captioned: “Just the two of us.”
