Emergency services were called to Canterbury Rd, near Ernest St, at Roselands at 3.30am following reports three cars had collided.
Police said the driver of a Land Rover, and sole occupant, fled the scene after allegedly crashing into a fence of a home, causing the vehicle to flip onto its roof.
The man attempted to run away from the crash site but police chased him down before tackling him to the ground.
He was arrested and taken to Canterbury Hospital for mandatory drug and alcohol testing.
Officers from Campsie Police Area Command have established a crime scene and are investigating the crash.
The driver of a second car was treated by paramedics at the scene before being taken to Bankstown Hospital for minor injuries, while the driver of another vehicle was uninjured.
