A knife-wielding man was “going ape” and hitting his girlfriend shortly before he was shot dead by Queensland Police, an inquest has heard.

The fatal shooting of Daniel Patrick Lewis, 36, occurred on August 31 at Norman Gardens, a suburb of Rockhampton.

A pre-inquest conference into the death was held in Brisbane Coroners Court today.

It heard his girlfriend’s son called triple-0 just after 7pm and reported Mr Lewis had “gone ape” and had hit the boy’s mother.

Counsel Assisting the Coroner, Josephine Villanueva, said the woman and her children left the home and waited on a neighbour’s driveway for police.

Arriving officers were told he may be armed and five minutes later Mr Lewis yelled he had a “shot gun loaded”, threatening police who had entered the backyard.

A short time later he walked out a side door armed with three knives and walked towards officers.

Ignoring calls to drop them and get on the ground Mr Lewis threw a knife at an officer but missed him, Ms Villanueva said.

The officer discharged his taser but this also missed Mr Lewis.

Another officer drew his firearm and repeatedly yelled for Mr Lewis to drop the knives and get on the ground.

Ms Villanueva said the officer then discharged his firearm shooting Mr Lewis twice.

Officers immediately provided first aid until paramedics arrived and took over the treatment but Mr Lewis could not be revived and was pronounced dead at the scene.

A post-mortem found he died as a result of gunshot wounds to his chest and abdomen.

The inquest will examine the actions of officers, their training along with police policies and procedures.

It is due to be held in Gladstone later this year.
