Emergency services were sent to the scene of the Cunningham Hwy crash at Gladfield about 2.40pm. A rescue helicopter was also sent.
A 17-year-old boy from Texas, in the Goondiwindi region, died at the scene.
Police said the boy was the driver and sole occupant of a ute that was travelling west on the highway when his vehicle collided with two oncoming vehicles near Gladfield Back Rd.
The occupants of the other two vehicles were taken to hospital for non-life threatening injuries.
A local farmhand, who asked not to be named, helped first responders shortly after the crash.
“It was a mess,” he said.
“One car was upside down in the middle of the road, another was on its side on the side of the road and there was a ute in the middle.
“It wasn’t pretty.”
The highway was closed in both directions for several hours.
