The 46-year-old man was treated for hypothermia by the Queensland Ambulance Service.
Crews were called to a bridge on Ryeford Pratten Rd that spans the Condamine River just after 6.30pm on Tuesday night.
Four swift water rescue teams assisted in the rescue, with crews knee-deep in water.
Emergency services had trouble locating the man, hearing voices at around 7.30pm but only gaining a visual of him an hour later at around 8.30pm, where he was found hanging onto a tree 30 metres downstream.
By 8.55pm the man was on dry land and was transported to Warwick Hospital in a stable condition.
