Police have confirmed the identity of a woman who died in a three-car horror smash on a major southern Sunshine Coast road on Monday.

Forensic Crash Unit investigations are ongoing after a 45-year-old Fitzgibbon woman died in the crash at Meridan Plains about 8.45am Monday, February 20.

A Queensland Police statement said initial investigations indicated a Kia Sorrento station wagon travelling north on Kawana Way Link Rd collided with a Toyota Hilux ute and trailer parked on the left shoulder of the road.

The collision sent the Kia into the path of an oncoming Honda Jazz driven by the 45-year-old woman.

The woman was rushed to Sunshine Coast University Hospital in a critical condition but tragically died in hospital from her injuries.

The 25-year-old woman driving the station wagon sustained minor injuries and was transported to hospital as a precaution while two men in the Hilux avoided physical injury.

Firefighters said one person had been entrapped as a result of the crash and they were freed from the vehicle about 9.10am.

Forensic Crash Unit investigations are ongoing and police have appealed for anyone with information or dashcam footage to come forward as they work to determine the cause of the crash.
