An investigation is under way and all site operations have been suspended.

It is the seventh mining fatality in Queensland in 18 months.

Fitzroy CEO Grant Polwarth said: “We are all extremely saddened by this tragic

incident and we extend our deepest sympathies to Brad’s family and friends.”

“Brad was a long-term Carborough Downs employee and a valued member of the Fitzroy team.

“Fitzroy’s current focus is on supporting Brad’s family, friends, and all of the Fitzroy workforce impacted by this event.”

Friends of Mr Duxbury said he was a “proud” father who was reserved but “passionate”.

An avid athletics enthusiast, Mr Duxbury devoted his spare time to the Ipswich and District Athletics Club.

Club president Vic Pascoe said the death had shocked the local athletics community.

“We were close friends, he was a proud dad of Alex who I coached in sprints for a number of years,” Mr Pascoe told The Courier-Mail.

“He was a very hard worker and put his all in whatever he did. Brad was a quiet and reserved man but followed my athletes with me all over Australia for Australian Championships, every capital city, so passionate about athletes doing well.”

A “highly regarded electrician” Mr Duxbury grew dissatisfied with manual timing for sprinting. As a result he invented the world’s first laser stopwatch for sprint races.

At the time of his death the Stazer was patent pending.

Mr Duxbury served as a committee member at the athletics club from 2005 to 2016.

“We have a Winter Carnival each year and he sponsored the U/18yrs Boys and U/18yrs Girls professional races from 2009 till 2016,” Mr Pascoe said.
