Fortescue Metals Group’s famed operational culture has taken a body blow with the sudden exit of three top executives over an apparent failure to communicate the extent of problems with the company’s big ticket Iron Bridge magnetite project.

The management upheaval includes highly regarded chief operating officer Greg Lillyman and is the biggest at Fortescue since chief executive Nev Power departed three years ago after setting up the iron ore miner for a period of sustained success.
