Lachlan Carslake, 28, remembered for his ‘good heart’ after heartbreaking death at Nylastex in Edwardstown

A family is in mourning after a 28-year-old man was tragically killed at an Adelaide engineering firm, describing him as someone whose “goofiness could make anyone laugh”.

The grief-stricken family of a man killed after a workplace incident at an engineering firm in Edwardstown has paid tribute his “good heart, his kind soul and goofiness”, saying he will be “forever 28”.

Lachlan Carslake was just 28 when he suffered critical head injuries at his Nylastex Engineering Solutions workplace last week.

Emergency services were called to Erudina Ave last Wednesday about 12pm following reports Mr Carslake had suffered serious head injuries while operating machinery.

He was rushed to the Flinders Medical Centre and placed on life support surrounded by his family and “love of his life” Tahlia Benvegnu-Carslake until he sadly died on Saturday.

A GoFundMe already amassing more than $8000 to help support Mr Carslake’s family described him as “a fun-loving, kind and caring son, brother, partner and friend to many”.

“There won’t be a day when Lachlan isn’t remembered or missed whether it be for his gorgeous smile, his good heart, his kind soul or his goofiness that could make anyone laugh,” the post said.

“I shouldn’t have to be writing this but unfortunately life just isn’t fair, no one should go to work and never come home.

“Lachlan’s legacy lives on as his decision to be an organ donor has saved the lives of others.

“Funds raised will go towards (his family’s) living expenses in Adelaide while they endure the ongoing investigation and planning of his funeral.”

The operators of Nylastex, which was recently “shut down” by SafeWork SA after being issued statutory notices, spoke out on Wednesday about their “heartbreak” and grief.

A spokesperson for the company, which has been operating since 1990, told The Advertiser the family-run business is devastated and was thinking of the dead man’s loved ones.

“We are heartbroken by the passing of our friend and colleague and our thoughts and prayers are with his loved ones,” they said.

“We have been, and will continue to, work cooperatively with SafeWork SA as we endeavour to understand exactly what happened.”

SafeWork SA said it had issued the business with statutory notices that had effectively shut it down.

Inspectors attended the site and have started a full investigation.

“A number of statutory notices have been issued to the engineering business in relation to this matter,” a spokesperson said on Tuesday.

“The notices have effectively shut down the business at this location and expert engineers have been engaged to assess the plant involved in the incident.

“As this is an ongoing investigation, we are unable to provide further information at this time.”

On its website, Nylastex describes itself as an Australian-owned and operated “advanced precision” engineering company that opened in 1990.

The family-owned business has more than 40 employees at its Edwardstown factory, providing machinery processes and services to a wide range of industries including agriculture, space, defence and mining.

During the Covid pandemic, they worked closely with the Detmold Group to establish and launch a face mask production facility. Within 16 weeks, the factory was manufacturing 750,000 masks per day.

It has also worked on the Collins class submarines, according to the Defence SA website.

SA police confirmed they will be preparing a report for the coroner.
